Automotive Repair Vendors

Approved Auto Repair Facilities

The Department of General Services (DGS), Office of Fleet and Asset Management (OFAM) maintains an Automotive Repair/Service program on behalf of the DGS and most other State agencies. 

 The OFAM Request for Qualifications (RFQ), RFQ DGS-OFA-VQ-1 (PDF format), solicits vendors interested in providing automotive repair/services for state vehicles.  Please also print the following two forms to fill out and return with the forms from the RFQ:  the Drug-Free Workplace Certification, Std. 21 (PDF format); and the Payee Data Record, Std. 204 (PDF format).

A complete response to this RFQ will include the following documents:

  1. A response checklist (Attachment A from the RFQ).
  2. A Vendor Application Form, OFA 50 (Attachment B from the RFQ).
  3. A Vendor Data Record, STD. 204 (PDF format).
  4. A Drug-Free Workplace Certification, STD. 21 (PDF format).
  5. A photocopy of your current Business License, (Issued by your local city or county government).
  6. A photocopy of your current Bureau of Automotive Repair License.
  7. Certificates of Insurance as required on pages 6 and 7 of this RFQ.
