Along with other services the Department of General Services Blue Charge Card (GS971) is used for renting state-owned equipment from state garages. Valid charge card number formats are:
Charge cards may be ordered from the Office of Fleet Administration, 1700 National Drive, Sacramento, CA 95834. Use the Request for General Services Charge Card (OFA 58a) form.
 Agencies/Departments are responsible for determining their own appropriate system for the ordering and reporting of lost, stolen, or discontinued charge cards and communicating this system to employees.
